Teacher/Administrator Instruction Sheet

Note to person administering this questionnaire: This questionnaire is designed to provide baseline quantitative and qualitative data regarding student attitude and behavior in relation to the economic concepts and Keystone Economic Principles (KEP) presented in the Infusionomics™ certification process.  The same questionnaire should be administered at the end of the school year as a means to measure increased knowledge and understanding of the KEP’s and evaluate the program as a whole.  It is important to remember that this is not a teacher evaluation tool.  Please read the following instructions verbatim to your students in order to prepare them to answer each question to the best of their ability.  Feel free to provide clarity in your own words as you see fit.

“Today you are going to take a personal reflection survey related to the economic literacy curriculum that I/we will be introducing this school year.  You will take the same survey at the end of the school year as well.  Keep in mind that this is not a test that you will be graded on.  The results will help us evaluate the effectiveness of this approach to help students become better decision makers.  The survey should take about 30-45 minutes to complete.  Read each sentence and answer the question “How much does this sentence describe me?”  Circle the number that matches your answer.  If the sentence does not describe you at all then circle “0”.  If the sentence describes you completely then circle “4”.  If your answer is somewhere in the middle then circle the number that best describes you.  Remember there are no right or wrong answers.”
